FIRST VISIT

TO GAIN THE GREATEST BENEFIT FROM YOUR FIRST APPOINTMENT:

Review your PATIENT ORIENTATION PACKET, and jot down any questions so that we can be sure to answer them for you.

Complete your PATIENT HISTORY FORM.

Bring your ORTHODONTIC RETAINER or ORAL APPLIANCE, if you are using one.

Bring your INSURANCE CARDS, medical and dental.

Plan to ARRIVE ON TIME and be with us for 2 HOURS, the time it takes for us to conduct a thorough examination.

BRING A FAMILY MEMBER OR TRUSTED FRIEND with you, especially if that person is involved in helping you make decisions about your healthcare. (An extra set of ears can be useful. We provide a great deal of information!)

RELAX! The examination is completely painless and we’re here to help you every step of the way.

GOALS: We value the trust you have shown in us by scheduling an appointment for a new patient examination during which we will:
  • Listen to, understand and appreciate your concerns
  • Gather and review data, and conduct a clinical examination
  • Present a preliminary diagnosis
  • Review options for treating causes (not just symptoms)
  • Outline a treatment plan and schedule and,
  • Discuss costs and a payment plan that fits in your budget


ABOUT OUR PATIENT HISTORY FORM:

Please complete your Patient History form in advance, and bring it with you to your appointment. We know that nobody likes to fill out forms. However, thorough and complete answers are important!

THE EXAM: A Treatment Coordinator who will take your picture and blood pressure, and review your medical history, will greet you. Next, your doctor will discuss your history and concerns in detail. The Treatment Coordinator will then gather additional data (including a number of measurements) and your doctor will complete a Doppler sonogram of the arteries and joints in your face. Once all data has been gathered and reviewed, and your clinical examination has been completed, your doctor will present a preliminary diagnosis and discuss treatment options with you. Success rates, scheduling, cost and payment options will also be discussed. We want you to be well informed, so that you can decide to proceed with treatment with confidence.

PLEASE BE ON TIME: Your first appointment will take 2 hours. We respect your time and do our best to stay on schedule. Your promptness aids us in meeting this goal for all of our patients.

If treatment is warranted, and you’d like to begin right away, we will try to complete additional records while you’re here – minimizing your time away from home or work. These may include diagnostic x-rays and photographs, impressions of your teeth, plus a bite registration.
